About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality hospice and home health care to senior patients. This role focuses on patient comfort, dignity, and individualized care while working closely with families, caregivers, and interdisciplinary care teams.

Requirements

Active Registered Nurse (RN) license
Strong assessment, clinical, and communication skills
Compassionate, patient-centered approach to care
Ability to work independently in home-based settings
Valid driver’s license and reliable transportation
CPR/BLS certification

Nice To Haves

Hospice or palliative care experience
Experience working with geriatric patients
Knowledge of Medicare regulations and documentation standards

Responsibilities

Provide skilled nursing care to hospice and home health patients in their homes or senior living environments
Assess patient conditions and develop, implement, and evaluate individualized care plans
Administer medications, treatments, and wound care as ordered by physicians
Monitor patient symptoms, pain levels, and overall health status
Educate patients and families on care plans, medications, and symptom management
Collaborate with physicians, social workers, aides, and other healthcare professionals
Maintain accurate and timely patient documentation and reports
Provide emotional support and compassionate care to patients and families
Follow all state, federal, and company policies and healthcare regulations
