The Registered Nurse (RN) plays a vital role in providing comprehensive, patient-centered care within our multidisciplinary health team. This position supports CFHC’s mission by delivering high-quality, evidence-based nursing care; coordinating health services across acute, chronic, and preventive care; and fostering long-term wellness through health education and patient advocacy. Working collaboratively with medical providers, behavioral health, and care coordination staff, the RN ensures that each patient receives personalized support — from maternity care and family planning to chronic disease management and health promotion. The RN will perform clinical assessments, dispense prescribed birth control per protocol, manage care transitions, and coach patients toward self-management goals. This role also emphasizes proactive engagement with patients and families, integrating home visits, case conferencing, and data-driven quality improvement initiatives.

Requirements

  • Three-year diploma, two-year Associate, or Bachelor’s degree in Nursing; Bachelor’s degree in Nursing preferred.
  • As appropriate to education, maintain the following clinical licensure and certifications without restriction, probation, or limiting conditions or threats thereof, including meeting all requirements for ongoing training and supervision: Registered Nurse (WAC 246-840).
  • Two years of nursing experience, preferably in an outpatient setting.
  • Clinical care management experience.
  • Knowledge of evidence-based preventive health, chronic disease, and primary care.
  • Solid background in patient education and lifestyle risk reduction.
  • Experience using and managing electronic health records systems; analyzing and using data from EHR systems to improve quality; optimizing use of EHR.
  • Demonstrated ability to work with a low-income, multi-ethnic patient population and relate respectfully and effectively to people of diverse racial, educational, and socio-economic backgrounds.
  • Must be a confident, self-directed individual who can work within a team as well as independently.
  • Strong communication, organization, documentation and record keeping skills necessary.
  • Ability and willingness to be cross-trained and function in other support roles in the clinic.
  • Valid, unrestricted driver’s license required.
