About The Position

SUMMARY: POSITION: CHARGE NURSE SUPERVISOR: DIRECTOR OF NURSING JOB DESCRIPTION The Charge Nurse provides holistic care for residents to include assessment, planning of care, use of appropriate nursing interventions and the evaluation of resident outcomes. Cooperative work is an expectation of the job. Verbal and written communication of resident condition will reflect significant physical and behavioral changes to other members of the team. The Charge Nurse is responsible for the provision of care and safety of residents during his/her shift, administers medications and treatments as prescribed by the physician, provides teaching for both resident and staff, appropriately documents and reports all incidents/accidents occurring in the facility, maintains confidentiality of resident information, and acts as a role model for other staff members.

Requirements

  • A graduate of an accredited school of nursing and currently licensed to practice in the State of Texas.
  • Possesses current basic life support document (BLS-C) from the American Heart Association.
  • Demonstrates a negative criminal background check and drug screen.
  • Demonstrates a normal physical exam and reference review.
  • Demonstrates ability to lift 50 pounds.
  • Communicates in a positive manner with team members, elders, and their family members.
  • Demonstrates the ability to work in a cooperative manner with others.
  • Possesses capability to work alternating shift assignments.
  • Demonstrates personal cleanliness.

  • Utilizes standard precautions in the delivery of resident care.
  • Utilizes protective gear appropriately.
  • Performs accurate physical assessment(s) of residents.
  • Demonstrates understanding of the resident’s disease process, illness trajectory, medical and nursing care.
  • Proactively makes rounds to identify changes from residents’ psychosocial and physical baseline, safety risks, infection control risks each shift.
  • Completes required documentation for admissions, transfers, discharges and deaths.
  • Initiates (RN) careplan upon admission and modifies (RN, LVN) care plans to reflect changes in residents.
  • Recognizes significant changes in residents, notifies and documents physician and family member/responsible party communication.
  • Utilizes nursing interventions to meet the psychosocial, physical, and spiritual needs of residents.
  • Evaluates resident outcomes and effectiveness of intervention.
  • Safely administers medications and treatments.
  • Obtains consents for bed rails, TB screening, immunizations, psychiatric medications, DNR prior to implementation.
  • Prepares incident or accident reports on residents, employees, and visitors as required.
  • Appropriately intervenes to prevent/delete hazardous conditions within the environment.
  • Supervises direct resident care personnel to ensure the provision of total care.
  • Teaches resident, family members and staff to promote the highest level of resident wellness and function.
  • Performs drug counts of controlled substances each shift.
  • Conducts interdisciplinary rounds and shift reports.
  • Orders and re-orders resident medications as needed.
  • Participates in staff meetings and in-services.
  • Documents counseling sessions of direct resident care personnel, providing the original form to the DON and a copy to the employee.
  • Effectively responds to emergency situations.
  • Other duties as assigned by management.
