About The Position

Community Care Hospice is a dedicated hospice care organization committed to providing compassionate end-of-life care for patients and their families. Serving our rural communities, we believe in delivering quality care that meets the unique needs of each individual. We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team as a Case Manager. In this role, you will coordinate and deliver skilled nursing care and end-of-life support to patients and their families in home and facility settings.

Requirements

• Current unrestricted State of Kansas License to practice as a Registered Nurse
• Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations
• Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment
• Must be self-directed, motivated and flexible
• Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Community Care Hospice policy
• Availability of reliable transportation is required for this position

Nice To Haves

• Home Health or Hospice experience preferred
• One year professional medical-surgical experience preferred

Responsibilities

• Assess patient needs and develop individualized care plans.
• Educate patients and families on hospice philosophy, comfort measures, and medication management.
• Monitor and document patient conditions, adjusting care plans as needed.
• Participate in on-call rotation and provide after-hours coverage as needed.
