Responsibilities
- Developing and maintaining individualized client care plans
- Completing supervisory visits and in home assessments
- Monitoring client condition and recommending care adjustments
- Providing hands on caregiver training
- Ensuring caregiver competency with delegated tasks
- Reassessing clients every 90 days or sooner if needed
- Documenting changes in condition and updating care plans
- Communicating clearly with clients, families, and the care team
