About The Position

We are seeking a full-time Registered Nurse
to join our Dual Diagnosis Treatment
Team  based out of Pottsville, PA.

The full-time registered nurse (RN) plays a pivotal role in our community-based
Dual Diagnosis Treatment Team (DDTT), serving the Northeastern PA region. The
dual diagnosis team takes a compassionate, recovery-oriented approach dedicated
to providing high-quality care and unwavering support for individuals diagnosed
with both a mental illness and an intellectual disability. The RN will spend
their time in the community, directly interacting with consumers in their homes,
traveling, and documenting their work.

Requirements

• A degree from an accredited school of nursing required.
• Active and unrestricted Registered Nurse (RN) license and eligible to practice in Pennsylvania.
• Active and valid driver's license required.
• Knowledge and understanding of recovery principles and the recovery driven service system.
• Understanding of adults with serious mental illness and/or intellectual developmental disabilities.

Responsibilities

• Collaborate with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
• Perform physical health medical monitoring, assessment, etc.
• Support implementation of intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
• Complete accurate documentation in the electronic medical record.
