About The Position

Elder Services of Cape Cod & the Islands (ESCCI) is a nonprofit, community-based organization dedicated to promoting welfare, enhancing the quality of life, and maintaining the dignity of elders in Barnstable, Dukes (except Gosnold), and Nantucket Counties. The organization serves as a central source for information and referral, an advocate, collaborator, catalyst, coordinator of services and care, an educator, and a provider of direct services. This opportunity is for a Registered Nurse looking for meaningful work in a supportive environment, joining a team that values its employees. The role involves conducting assessments and screenings for Home Care, CAE, and other agency programs, consulting with agency staff, and developing care plans as needed.

Requirements

• Excellent computer skills required
• Ability to communicate and work with the elderly and community resources serving the elderly population
• Ability to work independently and professionally
• Valid MA driver’s license and car required
• Graduated from an accredited School of Nursing
• Valid RN License
• Minimum of one-year clinical experience in community health and/or long-term care

Responsibilities

• Conduct assessments and screenings for Home Care, CAE, and all other agency programs
• Have consults with agency staff as necessary
• Develop care plans as needed
