About The Position

The Registered Nurse (RN) provides high-quality, patient-centered nursing care within Camino Health Center’s clinical operations. This role focuses on direct patient care, clinical triage, coordination of services, patient education, operational support to providers and care teams, and collaboration with the laboratory department to ensure timely and accurate processing of patient specimens. The RN also collaborates with Population Health and Quality Improvement departments to support clinical protocol development, back-office standard work, and population-based care initiatives. The RN contributes to a culture of safety, service excellence, and continuous improvement. Requirements

  • Bachelor of Science in Nursing (BSN) required.
  • Minimum of two years recent clinical experience
  • bilingual English/Spanish required
  • Current CA RN License
  • current BLS Certificate

Responsibilities

  • Performs real-time triage (telephone, patient portal, walk-ins, nurse advice line, in-person) following established protocols.
  • Supports walk-in workflows, including rapid assessment, prioritization, and coordination with providers and back-office teams.
  • Administers medications (oral, injectable) and performs blood draws or injections as needed based on provider orders and clinic protocols.
  • Supports laboratory operations, including specimen collection, labeling, tracking, and communication of results as appropriate.
  • Provides patient education in both individual and group settings.
  • Performs wound care, suture removal, and other nursing procedures following standard protocols.
  • Communicates abnormal lab results, follow-up needs, health education instructions, or community resource recommendations as directed by providers.
  • Submits Confidential Morbidity Reports per regulatory requirements and provider direction
  • Calls in prescription refills as directed by medical providers
  • Monitors and maintains clinic emergency equipment monthly (oxygen, emergency kit, AED).
  • Supports development of clinical protocols, nursing workflows, and back-office standard work.
  • Works closely with providers to support efficient management of clinical queues and patient flow.
  • Collaborates with Population Health and Quality Improvement teams to deliver coordinated care for high-risk patients.
  • Uses population health strategies to support chronic disease management and reduce avoidable ER utilization.
  • Coordinates with hospitals and care teams to ensure timely post-ER and post-hospital follow-up.
  • Conducts outreach to new patients following health plan and CalOptima guidelines.
  • Supports HEDIS and quality improvement activities.
  • Participates in remote patient monitoring workflows, including education, follow-up, and data review.
  • Supports follow-up for Breast and Cervical Cancer Treatment Program patients to ensure timely documentation and care coordination.
  • Coordinates referrals, authorizations, community resource connections, and follow-up care.
  • Maintains strong working relationships with specialty care providers, hospitals, health plans, and community partners.
  • Collaborates with QI and operational leaders to improve clinical workflows, care coordination processes, and patient navigation.
  • Participates in departmental meetings and cross-functional workgroups.
  • Assists with specimen collection, processing, and transport to ensure accurate and timely lab results.
  • Collaborates with lab personnel to identify workflow improvements and support quality control initiatives.
  • Provides education to patients regarding lab procedures and preparation.
  • Demonstrates compassionate, patient-centered communication in alignment with Camino Health Center’s values.
  • Works cooperatively with providers, staff, volunteers, students, and patients using culturally sensitive communication.
  • Upholds all policies, procedures, and clinic standards.
  • Maintains strict patient confidentiality.
  • Performs other duties as assigned by supervisor
  • Participates in workgroups, committees, and improvement projects as needed
  • Travel between clinic sites as directed; you will be reimbursed for mileage across clinics
