About The Position

The Registered Nurse (RN) at Triple C Housing plays a vital role in supporting individuals with mental health needs through comprehensive health assessments, care planning, and wellness monitoring. This position involves collaborating with a multidisciplinary team to develop individualized recovery plans, ensuring compliance with state regulations, and promoting health education and maintenance. Requirements

  • Current and valid Registered Nurse (RN) license in the state of New Jersey.
  • 5+ years' experience with mental health and at-risk populations
  • Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN)
  • Strong knowledge of psychiatric meds and general health issues
  • Proficient in Microsoft Office
  • CPR and First Aid certified
  • Valid driver’s license; clean 3-year driving record

Responsibilities

  • Conduct intake, reassessment, and discharge nursing evaluations.
  • Participate in the development and approval of Individual Recovery Plans.
  • Ensure compliance with DMHAS, CMS, and agency policies.
  • Perform face-to-face health monitoring every 60–90 days.
  • Stay current with regulatory changes and participate in audits and inspections.
  • Supervise medication management and chart documentation protocols.
  • Provide health education to consumers, families, and staff.
  • Evaluate and respond to immediate medical needs and recommend interventions.
  • Collaborate with treatment teams and participate in case reviews.
  • Review and advise on medication regimens in coordination with prescribers.
  • Support wellness and recovery through evidence-based practices.
  • Maintain relationships with healthcare providers and community partners.
  • Participate in on-call rotation as needed.
  • Document care plans, progress notes, and billing information accurately and timely.
  • Prepare reports and contribute to quality improvement initiatives.
  • Supervise and support clinical staff as assigned.
  • Complete required training and additional assignments as directed.
