About The Position

The Registered Nurse is responsible for overseeing all aspects of medical care for those individuals for whom RHD provides services.

Requirements

  • Current PA RN/LPN licensure
  • Current CPR/First Aid certification
  • Valid Driver’s License
  • Act 33/34 clearances
  • FBI clearance as needed
  • All trainings required by licensure
  • Must complete a Physical, TB test, and Hep B titer.
  • Proficient use of computers, mobile devices, and agency software.
  • Business use of company equipment including telephone and vehicle.
  • Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.

Responsibilities

  • Complete daily mental status exams on 6–12 residents.
  • Complete narcotic counts at the beginning and end of shift.
  • Administer medication via the medication administration record.
  • Identify early signs of psychiatric relapse, medication side effects, or medical issues.
  • Maintain detailed nursing notes in EMR/charting.
  • Document progress toward treatment plan goals.
  • Ensure compliance with state regulations, Joint Commission, and organizational policy.
  • Lead response to psychiatric or medical emergencies.
  • Make patient appointments and assist in transportation when needed.
  • Collaborate on Personal Recovery Plans which emphasize residents’ strengths and needs.
  • Engage with residents regularly to note changes that may impact the recovery plan.
  • Participate in program milieu.
  • Attend and/or facilitate educational groups and meetings.
  • Provide education to support residents in understanding medication management.
  • Provide education to support residents in understanding daily health and wellness needs.
  • Complete assessments/physicals within regulatory time frames.
  • Ensure residents’ prescriptions are current and collaborate with MD as needed.
  • Coordinate medications and health needs with the program Psychiatrist.
  • Maintain electronic medication administration record (eMAR).
  • Monitor the administration of all medications.
  • Assist residents with scheduling medical appointments and follow up on recommendations.
  • Coordinate physical healthcare with Primary Care Physicians and specialists.
  • Document educational groups and other resident contact.
  • Participate in case consultations, staff meetings, and supervision as assigned.
  • Attend training as required by RHD and licensing bodies.
  • Perform duties as assigned by director to ensure continuity of care.
  • Patient Care: RNs assess patient health, develop care plans, and implement nursing interventions to ensure optimal patient outcomes.
  • Medication Administration: Administer medications and monitor patients for side effects and reactions, ensuring adherence to prescribed treatment plans.
  • Collaboration: Work closely with physicians and other healthcare professionals to coordinate patient care and communicate updates regarding patient conditions.
  • Documentation: Maintain accurate and complete patient records, documenting vital signs, symptoms, and changes in patient conditions.
  • Patient Education: Educate patients about health conditions, treatment options, and self-care strategies to promote recovery and wellness through individual and group sessions.
