Registered Nurse ensures the delivery of high-quality, person-centered medical care that supports the health, safety, and well-being of people supported in Developmental Disabilities Division. The RN promotes optimal health outcomes through assessment, care planning, coordination of medical services, and staff training and oversight.

Requirements

Current New York State Registered Nurse (RN) License in good standing (required).
1+ years of nursing experience.
Basic Life Support (BLS) certificate.
Demonstrated strong commitment to safety.

Nice To Haves

Experience working with individuals with developmental disabilities in residential or community-based settings (strongly preferred).
Strong clinical assessment skills to identify, monitor, and respond to medical conditions and changes in health status.
Ability to educate and train staff on health-related topics, infection control, emergency protocols, and individualized medical needs.
Strong organizational and documentation skills, with the ability to maintain accurate health records and support compliance with OPWDD standards.

Responsibilities

Registered Nurse ensures the delivery of high-quality, person-centered medical care that supports the health, safety, and well-being of people supported in Developmental Disabilities Division.
The RN promotes optimal health outcomes through assessment, care planning, coordination of medical services, and staff training and oversight.
Assess persons supported medical conditions to develop targeted care plans that prevent illness, manage chronic conditions, promote long-term well-being and advocate for the rights and well-being of individuals served.
Conduct regular health assessments and nursing reviews.
Administer medications and treatments as prescribed; monitor for adverse reactions and report appropriately.
Track and analyze health trends, hospitalizations, and incidents to proactively address health concerns.
Coordinate with physicians, DPSs, families and other healthcare providers to align care efforts and reduce gaps in medical management.
Provide prompt and accurate telephone triage and consultation to persons supported and staff, ensuring urgent health concerns are quickly identified and effectively managed to prevent complications.
Working with the care team to reduce avoidable hospitalizations or ER visits.
Ensure accurate, timely, and thorough documentation of all nursing activities.
Maintain compliance with state and federal regulations (e.g., Medicaid, OPWDD, CMS, HIPAA).
Participate in audits, surveys, and quality assurance initiatives.
Train and oversee unlicensed staff (e.g., DSPs) in medication administration, health monitoring, infection control, and emergency protocols.
Provide AMAP certification and recertification training, and monitor certified staff to uphold medication administration standards.
Maintain Medication Safety: Oversee medication administration processes to ensure compliance with regulations and minimize errors.
