Registered Nurse ensures the delivery of high-quality, person-centered medical care that supports the health, safety, and well-being of people supported in Developmental Disabilities Division. The RN promotes optimal health outcomes through assessment, care planning, coordination of medical services, and staff training and oversight.

Requirements

  • Current New York State Registered Nurse (RN) License in good standing (required).
  • 1+ years of nursing experience.
  • Basic Life Support (BLS) certificate.
  • Demonstrated strong commitment to safety.

Nice To Haves

  • Experience working with individuals with developmental disabilities in residential or community-based settings (strongly preferred).
  • Strong clinical assessment skills to identify, monitor, and respond to medical conditions and changes in health status.
  • Ability to educate and train staff on health-related topics, infection control, emergency protocols, and individualized medical needs.
  • Strong organizational and documentation skills, with the ability to maintain accurate health records and support compliance with OPWDD standards.

Responsibilities

  • Registered Nurse ensures the delivery of high-quality, person-centered medical care that supports the health, safety, and well-being of people supported in Developmental Disabilities Division.
  • The RN promotes optimal health outcomes through assessment, care planning, coordination of medical services, and staff training and oversight.
  • Assess persons supported medical conditions to develop targeted care plans that prevent illness, manage chronic conditions, promote long-term well-being and advocate for the rights and well-being of individuals served.
  • Conduct regular health assessments and nursing reviews.
  • Administer medications and treatments as prescribed; monitor for adverse reactions and report appropriately.
  • Track and analyze health trends, hospitalizations, and incidents to proactively address health concerns.
  • Coordinate with physicians, DPSs, families and other healthcare providers to align care efforts and reduce gaps in medical management.
  • Provide prompt and accurate telephone triage and consultation to persons supported and staff, ensuring urgent health concerns are quickly identified and effectively managed to prevent complications.
  • Working with the care team to reduce avoidable hospitalizations or ER visits.
  • Ensure accurate, timely, and thorough documentation of all nursing activities.
  • Maintain compliance with state and federal regulations (e.g., Medicaid, OPWDD, CMS, HIPAA).
  • Participate in audits, surveys, and quality assurance initiatives.
  • Train and oversee unlicensed staff (e.g., DSPs) in medication administration, health monitoring, infection control, and emergency protocols.
  • Provide AMAP certification and recertification training, and monitor certified staff to uphold medication administration standards.
  • Maintain Medication Safety: Oversee medication administration processes to ensure compliance with regulations and minimize errors.
