Requirements

  • Graduate of an accredited school of professional nursing
  • Current license as a Registered Nurse (RN) in the state(s) of practice
  • CPR certification
  • Reliable Transportation/Travel is REQUIRED
  • Providing patients, families and direct care staff regarding non-medical treatment options.
  • Strongly assess skills/Required
  • Must be proficient in Microsoft Office suite and other EMR's for adult care communities

Nice To Haves

  • 2 years Home Care or Home Health experience/Preferred
  • 1 year case management experience/Preferred

Responsibilities

  • Completing 90 day regulatory maintenance of supervisory visits and chart audits
  • Display exceptional social-emotional intelligence in executing care, training and supervison of staff
  • Providing interventions with a focus on achieving realistic client outcomes.
  • Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
  • Maintaining client records showing systematic assessments, planning interventions, and evaluations.
  • Orienting new hires and over seeing CNA's
