About The Position

Lathrop Community Health Services Department is seeking to fill a Registered Nurse position; a 24 hour-per-week, part-time Registered Nurse, available days and every other weekend to join our capable and friendly team of employees dedicated to helping enhance the experience of aging. The Registered Nurse position will direct and coordinate resident care, while serving as a health and wellness educator, advisor and advocate to both the resident and the family. The position is responsible for: Delivering safe and comprehensive resident care (utilizing assessment, evaluation, planning and implementation) Demonstrating clinical judgment and decision making for the aging population Promoting ongoing health and wellness programs to residents Ability to recognize and respond to emergency situations Developing and implementing resident plans of care Coordination of resident services Working collaboratively with outside agencies to facilitate care/services

Requirements

  • Current professional licensure in MA - Registered Nurse
  • Current CPR certification
  • 3-5 years’ experience focusing on older adults in a homecare, ambulatory care or Assisted Living setting
  • Strong clinical assessment and case management skills
  • Exceptional organizational skills, flexibility to manage and prioritize multiple tasks
  • Excellent and effective interpersonal and communication skills
  • Ability to work independently or within the team
  • Working computer knowledge (Microsoft Office, Email)
  • Must be able to read, write, and speak English
  • Must possess a valid driver’s license and reliable transportation
  • Ability to pass a criminal background check (CORI) and pre-employment health requirements
