About The Position

The Registered Nurse under the direction and supervision of the Clinic Manager, provides skilled nursing care to participants in the PACE clinic and in participant’s homes, implements primary care orders, evaluates participant symptoms in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team. Senior Community Care of North Carolina - PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization. PACE is a Program of All-Inclusive Care for the Elderly. Requirements

  • Graduate of an accredited school of professional nursing
  • Current state Registered Nurses (R.N.) License
  • Cardiopulmonary Resuscitation (CPR) certification required
  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Must have a valid driver’s license, proof of insurance and have means of transportation
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact

  • Bachelors of Science Nursing (BSN) preferred.

Responsibilities

  • Conducts comprehensive initial nursing assessments in the PACE clinic and in participant’s homes with new PACE enrollees.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person nursing re-assessments in the PACE clinic and in participant’s homes semiannually and as needed.
  • Participates in the nursing on-call rotation.
  • Functions as a member of the Interdisciplinary Team.
  • Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • In collaboration with the Home Care Manager, completes Supervisory Visits of Home Care Aides in participant’s homes per policy.
  • As needed provides orientation, initial and annual competencies of Licensed Practical Nurse (LPN), Certified Nursing Assistant’s (C.N.A.’s), and Clinic Support staff.
  • Performs general nursing care to participants both in the clinic and in participant’s homes within the nursing scope of practice
