Requirements
- Active RN License in Washington State
- Team player with a great attitude and a caring heart
- Reliable transportation and valid driver’s license
Nice To Haves
- Home Health expeirence preferred
- Willing to train the right candidate
Responsibilities
- Provide skilled nursing care to patients in their homes
- Develop individualized care plans in coordination with the physician and care team
- Complete accurate and timely documentation
- Educate patients and caregivers on disease management and care plans
- Collaborate with interdisciplinary teams to ensure quality patient outcomes
