As a Hospice RN / RN Case Manager, you will be the primary point of contact for patients and families, guiding them through every step of their hospice journey. You will deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care in their home, assisted living, or wherever they call home. This role involves performing initial and ongoing assessments, collaborating with an interdisciplinary team (physicians, LPNs, CNAs, social workers, chaplains, and others) to build and refine personalized care plans. You will provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and bereavement guidance. A key aspect is teaching caregivers how to safely and confidently care for their loved one, providing practical training and emotional reassurance. You will also communicate important updates to the care team, maintain accurate clinical documentation, and follow infection control and compliance standards. Additionally, you will support quality improvement and scheduling initiatives and help patients and families understand medication administration, hospice philosophy, symptom/pain management, and end-of-life processes.
