The RN educates patients and their family members and ensures the safety of the patient. The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state. As a Registered Nurse Admission, you will make a difference in the lives of our patients. As a Registered Nurse (RN) Admission you will have the opportunity to provide one on one patient care and work at the top of your license. You will coordinate care and provide home-based nursing care for patients as directed by an attending physician. Home care provides context and real-world perspective about what will really help patients restore their health. You’ll provide clinical care under the direction of the physician. You will educate patients and their family members and ensures the safety of the patient. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. Requirements

Registered Nurse with current license in the state of employment.
Minimum of one year of experience as an RN in an acute care setting.
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.
Ability to remain calm, have patience and be accommodating.
Compassionate and caring while working with patients.
Knowledgeable on nursing best practices.
Ability to make appropriate nursing judgments.

Home care experience

Responsibilities

Provide comprehensive skilled nursing care in a patient’s home as ordered by the attending physician.
Observe and monitor patient conditions.
Perform OASIS assessments to develop individualized plans of care and make adjustments as needs change.
Administer medication as prescribed by the physician.
Help decrease re-hospitalizations by front loading visits for high-risk patients and teaching preventative measures.
Manage multi-disciplinary care as applicable.
Promote continuity of care with appropriate admissions, transfers, and discharges.
Counsel patient and family on disease/injury processes and management, incorporating them into the plan of care.
Oversee and supervise total care of patients provided by Aides and LPNs.
Evaluate treatments and medical condition of the patient according to the plan of care.
Determine if the level of care being provided meets the patients’ needs.
Communicate with LPNs and Aides on supervisory visit results.
Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines, including treatment, assessments, and patient education.
Maintain active RN license in the state of employment.
Communicate with the scheduler any changes outside the normal schedule/availability.
Participate in all on-call requirements and case conferences.
Attend in-service trainings and mandatory agency meetings.
Maintain a professional appearance as a representative of the company.
Understand and practice agency policies and procedures, rules, and regulations.
Drive to patient’s primary location per scheduled visit.
Ensure daily attendance at assigned visit locations.
Document all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours.
Document all aspects of start of care visits within 48 hours.
Complete and submit all required documentation within specified company requirements.
Follow the plan of care as permitted within the scope of practice for a Registered Nurse.
Fulfill RN visits in assigned geographic location per patient need within federal and state specifications.
Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
