ACT is an intensive, community-based multidisciplinary treatment team that provides person-centered care to a shared caseload of adults whose needs are not able to be met through traditional outpatient services. In collaboration with a psychiatric care provider, the team nurses assist with medication management and administration, through documentation as well as monitoring clients for medical problems or side effects. Nurses will communicate and coordinate services with other medical providers, engage in health promotion, prevention, and educational activities. The nurse will educate other team members to help them monitor psychiatric symptoms and medication side effect and work with clients to develop strategies to maximize the taking of medications as prescribed.
