The RN Concierge Member Care Coordinator is a key liaison between Concierge Program Participants, the Concierge Program (Physician(s) and other supporting staff, Intermountain Healthcare Services and Programs, and Referring Specialists. This role provides clinical support to the patients and provider(s) and manages patient relationships in a way that continue to grow overall member participation for Intermountain Concierge Services. This Nursing position provides patient care as well as Member Relations and Member Growth and Participation (Sales). The RN Concierge Member Care Coordinator will actively maintain and manage member relations, growth, and participation and will enroll new concierge members via clinic / patient contract outlined in-clinic procedures. Works to build and maintain relationships with program participants by effectively communicating in a positive manner with an attempt to meet the patients' needs as appropriate. Actively participate in member retention and help members with contract renewal and payment collection. The RN Concierge Member Care Coordinator will be available to answer and respond to patients' needs who call into the clinic seeking care and advice and will help field, triage, and schedule patient as appropriate based on patient need and physician direction. Is respond to patient questions timely as to provide a high-touch service and helps coordinate referrals to other physicians and specialists, Lab Services, Radiology, and other Ancillary Services. Responsible to perform clinical nurse duties including making appropriate assessment of physical, social, and psychological status (including cognitive, communicative, and developmental skills as appropriate); seeks and evaluates information acquired from other members of interdisciplinary team: patient (e.g., family, physician, non-nursing disciplines, support staff). Also responsible for planning, using assessment information and critical thinking skills to collaboratively develop individualized plan of care; actively seeks patient and family involvement to develop a plan of care; collaboratively plans and prepares patients for further support; prepares patients for further support needs once the current episode of care outcomes are met (i.e., teaching, referrals, and follow-up). Implements and directs the interdisciplinary care team via delegation, coordination, and collaboration and evaluates patient response to interventions through review of achievement of goals, clinical outcomes, patient and family satisfaction; revises plan of care as indicated and reassesses changes as appropriate. Actively participates in quality improvement initiatives (i.e., CQI, TQM, Lean, 100% participation, etc.).
