About The Position

The Btle Creek VA Medical Center is seeking a Registered Nurse to join the Wyoming PACT Team within the Ambulatory Care Nursing Service. This role involves providing direct care to Veterans, managing patient panels, and promoting health and wellness through education and coaching. The nurse will be responsible for developing care plans based on assessments and ensuring optimal care delivery in a primary care setting.

Requirements

• U.S. Citizenship; non-citizens may only be appointed when qualified citizens are unavailable.
• Graduation from an accredited school of professional nursing.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state or territory.
• Proficiency in basic written and spoken English.

Responsibilities

• Provide direct Veteran care and care coordination within the Primary Care Department.
• Manage assigned patient panels and ensure appropriate evaluation and access to care.
• Deliver health promotion, education, and coaching on wellness and disease prevention.
• Formulate care plans based on nursing assessments and patient needs.
• Delegate care tasks appropriately based on patient condition and safety.
