About The Position

We are seeking a skilled Wound Care Registered Nurse (RN) to provide specialized care for patients with wounds, ostomies, and continence-related conditions . This role focuses on comprehensive patient assessment, development of individualized care plans, and collaboration with interdisciplinary teams to improve patient outcomes.

Requirements

  • Minimum 2 years of Med/Surg nursing experience
  • Experience in wound care and colostomy care
  • Associate Degree in Nursing ( ASN/ADN ) from an accredited program
  • Active New York State RN License
  • BLS Certification (AHA)
  • Primary Source Verification through NYSED
  • One of the following certifications is required: CWOCN (Certified Wound Ostomy Continence Nurse) – WOCNCB CWON (Certified Wound Ostomy Nurse) – WOCNCB CWCN (Certified Wound Care Nurse) – WOCNCB CWS (Certified Wound Specialist) – ABWM WCC (Wound Care Certified) – NAWCO

Responsibilities

  • Conduct comprehensive health assessments focused on wound, ostomy, and continence care
  • Perform focused physical assessments and develop individualized care plans
  • Recommend appropriate treatments, therapies, and medications
  • Assess and document patient responses to wound care interventions, including complex wounds and fistulas
  • Collaborate with healthcare teams and initiate referrals as needed
  • Educate patients, families, and caregivers on prevention and management of wound-related conditions
  • Develop and deliver staff education programs, including orientation and ongoing training
  • Perform routine assessments for patients at risk of pressure injuries and implement preventive measures
  • Participate in quality improvement initiatives, including incidence and prevalence studies
  • Maintain accurate documentation and ensure compliance with confidentiality and patient rights policies
  • Assist in equipment and asset management (e.g., specialty beds, negative pressure wound therapy devices)
  • Promote culturally competent care and patient education
