Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients/residents with acute, chronic, or complex skin/wound care needs. Assesses, plans, implements, and evaluates care based on age-specific components. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The BT Wound Care Nurse (WCN) reviews inpatient wound consults and wound care nursing notes in the electronic patient chart daily. Provides discharge recommendations. Place and track all supply orders. Request specialty beds and request pickup of specialty beds through Prosthetic consults and VA Bed rental department. Coordinate on-going education for nursing staff and conduct post FAPI huddles with the inpatient nursing staff. Collect and aggregate HAPI, FAPI and CAPI data monthly. Monthly reports are sent to VAMHCS Leadership. Report and track new pressure injuries to Nursing Leadership. Collaborate with vendors to assess and review new products. Attend quarterly meetings for the Interprofessional Pressure Injury Committee (IPIC). Attend Bi-weekly meetings for Wound Ostomy Care Nurse shared practices, collaboration and updates. Resource for Outpatient Ostomy care, assist with ordering supplies, providing support to the veteran via phone calls and secure messaging.

Requirements

  • Licensure: 1 full and unrestricted license from any US State or territory
