Registered Nurse - Weekend Triage

About The Position

Regional Hospice is looking for licensed Weekend Triage RN to work with patients in the Community on the weekends. We offer a generous 401K Plan with a 4% Match for every 5% contributed with immediate vesting. This position needs a candidate who is skilled in pain and symptom management and is confident at making assessments over the phone. They need to be able to triage patient needs and assign other nurses (or self) to make patient visits as appropriate. This position may be structured as every weekend or every-other weekend, position. This role also could be full-time salaried position for ideal candidate willing to do weekend triage and mid-week nursing visits and would be eligible for our full benefits package including: Vision, Medical, Life, Dental, 401K Match, Vacation/Sick Time as well as the use of a vehicle for work and personal use.

Requirements

Graduation from an accredited school of nursing; BSN preferred
Currently licensed or the ability to obtain a license to practice as a Registered Nurse in the State of Connecticut.
A minimum of two years of nursing experience, preferably in medical/surgical nursing required
Valid Clean Driver’s License Required

Nice To Haves

Home Health Care and/or Hospice / End of life care experience preferred
Certified Hospice and Palliative Care Nurse (CHPN) preferred

Responsibilities

Triage patients for Hospice and Home Health Care in the community
Makes emergency visits to patients as needed
Provides Triage and Telephone nursing care to patients and families.
Analyzes physical, emotional psychosocial, environmental and spiritual needs.
Identifies potential problems based on triage
Completes and documents interactions as needed.
Communicates the patient’s plan of care to attending physician, other staff members, and outside agencies.
Follow up with patients based on notes from on call nurse.
