About the position

The Registered Nurse (RN) provides professional nursing services to individuals in their homes through a process of nursing assessment, planning, care, and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures, and the Nurse Practice Act. The RN carries primary care management functions for assigned patients and safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. The position may require specialized skills depending on the VNA assignment.

Responsibilities

  • Independently recognizes subtle changes in condition and modifies plan of care accordingly.
  • Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care.
  • Initiates emergency action in response to acute risk situations.
  • Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.
  • Evaluates patient care and promotes goal-directed change to achieve patient outcomes.
  • Discusses, documents, and applies Performance Initiatives and outcomes at the VNA level as they relate to the WellSpan Bold Goals.
  • Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients.
  • Initiates interdisciplinary collaboration in evaluation of patient progress.
  • Identifies and facilitates individualized patient needs for transfer/discharge and includes patient/family in decisions.
  • Begins reading current literature and expresses interest in evidence-based research and its application to best practice.
  • Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired.
  • Promotes an environment conducive to professional practice, interdisciplinary collaboration, and patient and staff safety.
  • Demonstrates leadership at point of care.
  • Provides leadership for problem solving and goal formation.
  • Promotes cooperation and communication among all team members.
  • Displays conflict resolution and negotiation skills to maximize team performance.

Requirements

  • Diploma Program Required or Associates Degree in Nursing, RN Required.
  • Bachelors Degree Nursing (BSN) Preferred.
  • 1 year Previous experience with the same patient population Required or Current VNA LPN transitioning to RN Required.
  • Licensed Registered Nurse Upon Hire Required or Registered Nurse Multi State License Upon Hire Required.
  • Basic Life Support Upon Hire Required.
  • Must possess a valid driver's license in the current state of residence.
