This unit is part of our new and advanced Virtual Connected Care™ Together Team nursing model that integrates experienced telemedicine RN’s into care assignments, thus helping with the workload. We offer a strong orientation program and opportunities for continued education and growth. Schedules include 8-hour Evening, 8-hour Night (or Rotate 8 E/N), 12D/12N Rotation, 12-hour Night and Baylor positions (higher base pay rate). This team is dedicated to patient care and each other. Unit D5 is a cardiovascular step-down unit caring for patients admitted for interventional cardiac procedures, medical cardiology, vascular surgery and the designated stroke unit. We serve patients with a variety of acute medical, cardiac, vascular and respiratory conditions. We monitor class I telemetry and manage cardiac drips. Specialized training includes CORE EKG classes and stroke certification. Requirements

  • Graduation from an accredited school of nursing and current licensure in the State of New York with at least 1 year of experience.
  • Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing preferred.
  • Maintains all licensures and certifications according to NYS regulations and specialty area requirements.
  • Participates in continuing education and updates and maintains knowledge and skills related to specific areas of expertise.

Nice To Haves

  • Specialty credentialing & educational degree according to clinical nursing practice specialty area preferred.
  • BSN required for level 4 RN hires.
  • GN’s or RN’s with < 1-year experience should apply to the Graduate Nurse Residency positions.

Responsibilities

  • Responsible for collaboration, communication & facilitation of optimal provision of care with assistance of computerized &/or digital platform that supports the daily management of care.
  • Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.
  • Utilize clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care.
  • Ensure a seamless, effective, efficient transition of care across the continuum.
  • Monitor quality metrics specific to the department & ensure complete & accurate documentation in the patient record.
  • Act as mentor, clinician, educator, consultant/collaborator, and navigator.
