About The Position

The Registered Nurse in the Triage Lakeville Primary Care team at Park Nicollet provides professional telehealth nursing services to patients and their families. This role involves independent nursing practice, utilizing a systematic approach to assess, plan, implement, and evaluate patient care in accordance with established guidelines and protocols. The nurse acts as a liaison between patients, physicians, and staff, advocating for patient needs while maintaining confidentiality and addressing concerns in a timely manner. The position also includes mentoring team members and fostering a collaborative work environment.

Requirements

• Graduate from an accredited nursing program resulting in an associate degree, diploma, or baccalaureate degree in nursing.
• Valid Minnesota Registered Nurse License.
• BLS provider status in accordance with American Heart Association (AHA) guidelines.

Nice To Haves

• One or more years of broad-based nursing experience, preferably in a clinic, hospital, urgent care, or transitional care setting.
• Telephone Triage Experience.

Responsibilities

• Provide telehealth nursing services to patients and family members.
• Utilize a systematic approach to nursing practice including assessment, planning, nursing diagnosis, implementation, and evaluation.
• Implement population-based standing orders to treat patients as appropriate.
• Act as a liaison between physicians, patients, and staff in delivering complex telephone care.
• Resolve patient concerns or complaints while maintaining a positive attitude.
• Mentor other team members and collaborate to identify and resolve departmental issues.
• Support a team environment that encourages clear communication and creative thinking.
