About The Position

Coordinate and facilitate patient admissions and transfers to other facilities within the market, other markets, and other healthcare facilities according to patient acuity, bed availability, and required services. Collaborate with the nursing staff and multidisciplinary team members to place patients in appropriate levels of care to achieve desired outcomes. Coordinate as liaison between physician requestors for consultants and specialty consultants. Assist in the care management process including pre-registration and insurance verification. Stay informed of patient changes and communicates the impact to admitting physicians. Review medical reports from referring hospitals or physician offices and relays information to the receiving nursing unit as needed. Develop and maintain relationships with referral sources and admission decision makers. Maintain standards for documentation and communications with physician/service referrals for legal, medical, statistical, and process improvement purposes.

Requirements

  • Registered Nurse obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
