About The Position

Riverside Nurse is seeking Registered Nurses to join their team of professionals, providing continuous quality care for patients without physical contact. The role involves assisting Riverside customers, patients, and the community through the Riverside Nurse line. The Transfer Center actively assists facilities/providers to safely get patients where they need to go, whether it's from one hospital to another or simply a safe transition to home or rehab. This is a Part Time-Day Shift onsite position, not remote. The nurse will work in collaboration with RHS Healthcare teams to assist with the transition of critical and non-critical patients 24/7 to Riverside Health System's Acute and Emergency facilities and services. Responsibilities include managing all inbound and outbound calls for critical transfers, performing patient symptom triage, coordinating patient transport based on clinical assessment, and facilitating appropriate patient bed placement.

Requirements

  • Education Program Graduate, Registered Nurse (Required)
  • 3-4 years RN Experience in Acute Care setting (Required)
  • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
  • Computer experience required
  • Efficient at typing/keyboarding required

Nice To Haves

  • Bachelors Degree, Nursing (Preferred)
  • 1 year Critical Care or emergency nursing (Preferred)
  • Transfer Center experience preferred
  • Nurse Triage experience preferred
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Preferred)

Responsibilities

  • Demonstrates ability to use clinical knowledge around clinical service lines, disease entities, and epidemiological issues that could affect transport, care, and placement.
  • Adheres to critical and non-critical protocols, algorithms, policies, and procedures.
  • Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
  • Performs critical and non-critical symptom-based triage through the Riverside Nurse line providing patient disposition from 911 to home care.
  • Demonstrates understanding of diverse patient care populations and modifies education methods to ensure patient/family comprehension.
  • Coordinates and communicates with transfer team members, medical service transport coordinator, physician, bed management, and Transfer Center medical director throughout the course of transfer or direct admission.
  • Promotes a network of other Riverside hospitals sending and receiving facilities to facilitate transfers efficiently and timely.
  • Communicates detailed information about patient status and demographic information when arranging critical and non-critical transport.
