About The Position

New England Baptist Surgery Center (NEBSC) is currently hiring for a full-time (40 hours per week) Registered Nurse – Total Join Nurse Navigator About us: New England Baptist Surgery Center is a state-of-the-art surgery center providing comprehensive orthopedic care for a range of ambulatory services, including total joint surgery and pain management procedures. Position Purpose: Facilitates all necessary care from the decision to have a joint replacement through the post-operative follow up. Serves as point of contact for physicians, patients, and caregivers to provide resources, education and assistance with accessing clinical and supportive care services offered within New England Baptist Surgery Center (NEBSC). Position Requirements: Current Massachusetts nursing licensure. Graduate of an accredited school of nursing. Orthopedic experience in an outpatient setting preferred. Ability to work at the bedside in PACU is required. Prior experience working as a pre-admission testing nurse preferred. Experienced nurse with minimum of 5 years of patient and clinical experience. Joint experience a plus. Proven track record of patient satisfaction and customer focus. Ability to interact with all levels. You must be proficient in computers and databases, research and statistics and comfortable with new technology. You must have superior interpersonal skills and the ability to multi-task in this fast-paced customer service environment. Skills and Abilities: Ability to correlate clinical data with patient’s medical and nursing care. Ability to set priorities and work independently with little supervision. Ability to take direction from others and provide information that is clear, concise and promotes efficiency on the unit. Ability to communicate well with Physicians, Patients, Co-workers, etc. Navigates patients through the continuum of care to eliminate barriers to health care and enhance patient and physician satisfaction. Functions as a collaborating member of the health care team, but also, must be able to act independently to assess patient and physician needs to ensure appropriate handoffs so that patients receive services promptly and efficiently. Applies the Nursing Process as appropriate within the context of the organization’s framework for professional practice and in accordance with guidelines established by the team. Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services. Follows patients throughout the care continuum, including preoperative appointments, home care, and outpatient Physical therapy. Visits patients in PACU during their stay. Reduces the risk for canceled surgical cases by ensuring the timing of all necessary clearances. Helps patients understand the need for durable medical equipment required for recovery. Develops concise patient itinerary for use by the patient and the care team and documents all communication with the patient. Develops, plans, and presents patient education program and tools. Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, anesthesia staff, diagnostic staff, nurses, social services staff, home care staff, etc. Collaborate with other health care team members to collect data for research, patient satisfaction, and incident tracking, which are all reportable to the Director of Nursing or designee. Flexible working hours. (Weekend/Holiday, day and evening responsibilities) The ability to correlate clinical data with patient’s medical and nursing care. The ability to set priorities. The ability to utilize a computer to create workflows, collect data, document findings, and communicate with patients, care givers, and surgical team members. The ability to provide information and direction to others in a clear, concise manner that promotes efficiency in obtaining required preop clearance, care within the ambulatory surgery center, and initial recovery at home. Essential Position Responsibilities: Job Knowledge Facilitates Total Joint Patient Education. Visit patients in PACU during their stay, reviewing patient education, Patient IQ and next steps. Review adverse event reporting for total joint patients, and follow up with patients, as needed. Report incidents, complications and infections from post op calls and Patient IQ Surveys through Healthcare Safety Zone. Collaborate with team members to collect data for research, patient satisfaction, and incident tracking, which are all reportable to the Director of Nursing and Quality and Infection Control Nurse Program Manager. Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services. Practice appropriate telephone and patient triage skills, being aware of the need to balance quality care with efficient use of time and resources. Perform verbal assessment with patient prior to admission – in person or via telephone, communicating information to appropriate team members. Ensure pre-operative care requirements of patients are accomplished through the use of the nursing process including assessment, planning, implementation, and evaluation. Perform patient teaching/education as established by the department. Assess pertinent information, plan, implement, and document interventions and evaluate results. Maintain and promote patient’s safety by assessing physical and psychological needs. Respond to telephone inquiries regarding expressed needs of the caller. Refer caller to appropriate resources including medical/ambulatory care, hospital services, and community services within established procedures and guidelines. Thoroughly document all call encounters utilizing manual and/or online information systems at the time of the call. Alert anesthesia of results of labs, physical assessment, or special needs of individual with medical problems prior to the date of surgery. Communicate appropriate information including general health information to surgeons and anesthesiologists concerning patient care. Communicate special physician’s orders and ensure patient charting is completed in an accurate, concise and timely manner. Assumes responsibility and accountability for effectively managing nursing care of individual patients as a team member. Evaluates patient’s responses to care based on effectiveness of nursing interventions/actions in relationship to established outcomes and consults with Medical Staff, Care Team Leader, or Nursing leadership regarding clinical variances and recommends changes to the plan of care. Incorporates discharge/home care planning into the multidisciplinary plan of care on an ongoing basis.
