Make a Difference Community Health Network has an excellent opportunity available with our Hospice Department. We are seeking nurses to commit to a 13-week assignment, working days. The Hospice Registered Nurse is responsible for case management of a group of hospice care clients, providing direct nursing care, and coordinating and supervising other disciplines involved to promote quality services to patients. The Hospice RN is responsible for providing skilled care to assigned patient according to the plan of care established by the patient’s physician. The Hospice RN practices within the scope of nursing as defined by the Nurse Practice Act. This position will cover the south/southeast which is I-70 south (some of Marion County and all of Johnson County). Would cover areas such as New Palestine, Beech Grove and Greenwood.

Requirements

  • Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC)
  • At least two full years of recent Hospice experience
  • Current BLS required
  • IN driver's license with valid insurance required

Responsibilities
