About The Position

The Incumbent is responsible for coordination of pediatric patient care from initial contact to discharge and subsequent postoperative outpatient care to ensure continuity and enhance the effectiveness of the Health Care Team in meeting patient care needs. The incumbent will support the physician/team in developing plan of care and advising both the patient and care team about patient's insurance status and options for care. Responsible for discharge and post-op planning and follow-up as well as the coordination of care across outpatient and inpatient settings. The incumbent will support clinical needs of the care team by obtaining authorizations and working with primary care physicians to obtain referrals, and provide patients' care coordination among community services (e.g. school social worker, physical therapist, insurance care manager, etc.). Prepares FMLA or home school request forms as necessary.

Requirements

  • Graduate of a nationally accredited nursing program and one year of professional nursing experience.
  • Licensed as a registered nurse in accordance with Chapter 464, Florida Statutes.
  • Demonstrates familiarity with collecting and preparing data analysis.
  • Articulate and has a good command of the English language.
  • Proficient in Microsoft Office, including Outlook, Excel, and Powerpoint.
  • Demonstrates ease with learning new programs and highly organized.
  • Ability to compose own correspondence.
  • Capable of working under pressure and producing within deadlines.
  • Ability to prioritize own work and proceed with minimum supervision.
  • Ability to maintain a professional approach at all times while interacting with team members and patients.
  • Ability to handle a variety of situations in a professional manner.
  • Current licensure by the State of Florida as a Registered Nurse.
