About The Position

The Registered Nurse Specialist functions as an integral member of the Children’s Multidisciplinary Assessment Team (CMAT). The individual in this position utilizes the nursing process to provide pediatric nursing expertise, assessment and evaluation services for children and youth in the determination of medically necessary long-term care services for Medical Foster Care (MFC), Skilled Nursing Facilities (SNF) and Model Waiver.

Requirements

  • Associates degree or higher in nursing from an accredited college or university.
  • Must hold an active Registered Nurse License in the State of Florida (F.S. Reference: 464) or Multistate RN License from another Compact State.
  • Must hold a valid, unrestricted Florida Driver’s License and have access to personal transportation.

Responsibilities

  • Reviews CMAT referrals for appropriateness and to ensure complete referral information has been obtained to start the CMAT process.
  • Follows up with stakeholders, as necessary, to obtain all needed documentation for referral.
  • Requests and follows up on request for medical records and clinical documentation.
  • Reviews clinical documentation and medical records received.
  • Completes nursing assessment interviews with necessary stakeholders to obtain information to complete nursing medical assessment.
  • Documents pertinent assessment information received and reviewed.
  • Completes comprehensive clinical assessments, ensuring family centered approach, within necessary timeframes.
  • Provides quality verbal and written communication with CMAT and community stakeholders to ensure understanding and engagement throughout process.
  • Facilitates coordination in the scheduling of CMAT staffing’s ensuring timely notification and engagement of necessary CMAT stakeholders including, but not limited to, core members of the CMAT team, the client’s Medicaid Managed Medical Assistance (MMA) plan and Agency for Health Care Administration (AHCA) or their designee.
  • Engages, advocates, and supports opportunities for parental participation and family-centered approaches in the CMAT process.
  • Provides nursing education, training, and technical assistance to CMAT and community stakeholders.
  • Participates in community outreach, education, and engagement of community partners and stakeholders to promote awareness of CMAT program and the referral process.
  • Provides Level of Care (LOC) training, from the nursing perspective, for new CMAT members within established timeframes.
  • Provides consistent messaging that aligns with statutes, rules, policies, procedures, and processes which affect decisions and determinations of eligibility for care and management of the client’s medical needs.
  • Advocates for consideration of clients being in the least restrictive setting, appropriate to their nursing medical needs, and the wishes of their parents or legal representatives.
  • Responds to regional and statewide requests for information about CMAT coordination and staffings.
  • Providing the nursing perspective, participates in the multidisciplinary team process to determine medically necessary eligibility for long-term care services of MFC, SNF, and Model Waiver.
  • Provides quality verbal review of completed written nursing medical assessment, ensuring family centered approach, and providing explanation of medical terminology or processes as needed at the CMAT staffing.
  • Contributes to staffing discussion, provides any additional nursing explanation of assessment information presented and answers questions as needed.
  • As a core member of the CMAT, contributes to collaborative team consensus building process for the determination of eligibility for medically necessary long-term care services.
  • Participates in the collaborative team consensus building process for recommendation of LOC for MFC and SNF consideration.
  • Participates in collaborative decision making and problem-solving strategies, with other members of the multidisciplinary team, to determine individualized services to the care and management of the nursing medical necessities of each client.
  • Supports opportunities for parental participation and family centered approaches in the CMAT process.
  • Completes additional CMAT activities within required timeframes.
  • Documents all CMAT activities in the clients’ electronic health record.
  • Responds to regional and statewide requests for information about CMAT coordination and staffings as well as raising the awareness of stakeholders regarding the team.
  • Facilitates access to reports and records concerning the client being staffed when proper consent is available.
  • Provides information from date base or other information systems as requested by management.
  • Documents and completes the outcome of the CMAT staffing through a summary process which includes any updated medical information presented at the staffing and any pertinent discussion points.
  • Summary includes CMAT recommendation for eligibility and, if applicable, LOC.
  • Disseminates CMAT staffing summaries to stakeholders according to outlined process and within required timelines.
  • Inputs, updates, and monitors client and program data in electronic documentation system and other necessary formats to ensure data reporting integrity and seamless coordination of information to relevant stakeholders.
  • Reviews data, assessments, documentation, and staffing summaries verifying accuracy and quality.
  • Participates in quality assurance and improvement needs.
  • Implements documented activities identified in the Plan, Do, Study, Act (PDSA) cycles.
  • Participates in training, evaluation, monitoring, and related follow-up activities to improve CMAT functions, activities, and outcomes.
  • Provides reports from data base systems as requested by management.
  • Includes coverage of MFC RN duties as assigned.
  • Participates in MFC on-call duties as assigned.
