The position is for a Registered Nurse Special Needs Coordinator to support the mission at Eielson AFB in AK for the families.

Requirements

  • Bachelor's of Science in Nursing (BSN)
  • BLS certification
  • A minimum of one-year full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.

Responsibilities

  • Oversees and manages the Exceptional Family Member Program Medical (EFMP-M) IAW DOD and Air Force (AF) policy.
  • Establishes and maintains procedures to identify family members with special medical and educational needs in a timely manner.
  • Uses clinical assessment skills to identify sponsors whose family members have special medical and educational needs.
  • Collaborates with other key service providers to ensure effective outreach and identification of special needs.
  • Ensures family members of AF personnel are appropriately Q coded (EFMP-enrolled).
  • Provides oversight for base-level data entry in AF provided data management systems.
  • Provides training to installation personnel and medical staff to support the implementation of EFMP-M.
  • Involved in the Family Member Relocation Clearance (FMRC) process and coordinates enrollment and travel screening.
  • Ensures every AF sponsor with family members with special needs has a SN file maintained at the Medical Treatment Facility (MTF).
  • Contacts Q-coded sponsors annually to determine unmet needs and request updates.
  • Collaborates with Integrated Delivery System (IDS) and other forums to provide family medical information and referrals.
  • Maintains a cooperative working relationship with base MPS, CSS, and AF Personnel Center (AFPC).
  • Participates in EFMP weekly care management reviews to discuss newly identified families and complex medical needs.
