About The Position

Registered Nurse (RN) - Senior Services | Primary Care Join a specialized Primary Care - Senior Services nursing team focused on supporting geriatric patients and their caregivers through coordinated, community-based care. This role is ideal for an experienced Registered Nurse with an interest in geriatrics, care coordination, and home-based patient support. Key Responsibilities Provide care coordination for geriatric and senior patients Deliver patient and caregiver education to support self-management of chronic conditions Conduct in-home nursing assessments in private residences, assisted living, and memory care communities Address acute patient concerns and caregiver needs in collaboration with providers Coordinate home health services, durable medical equipment, and community resources Support advanced care planning and discussions around goals of care Review and manage referrals for home visit and senior care programs Why Join This Team This multifaceted RN role offers close collaboration with primary care providers and community-based agencies to deliver holistic, value-based care. Nurses in this role play a key part in improving outcomes for older adults while supporting independence and quality of life. Role Overview The Registered Nurse (RN) is accountable for coordinating nursing care across the continuum, including direct patient care, patient and family education, and care transitions. Practice is guided by the Mayo Clinic Nursing Professional Practice Model and the ANA Nursing Scope and Standards of Practice. The RN plans, implements, evaluates, and communicates all phases of nursing care. This role includes opportunities for leadership through precepting, mentoring, quality improvement initiatives, and participation in committees or special projects. Patient care delegation is based on clinical judgment, patient acuity, and available resources.

Requirements

  • Active Registered Nurse (RN) license
  • Valid driver's license and personal vehicle required for home visits
  • Strong communication, teaching, and care coordination skills
  • Graduate of a baccalaureate nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). If graduated from a nursing program that was not accredited by ACEN, CCNE, or NLN CNEA, at least one year of RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic is required.
  • All entry-level associate degree registered nurses with a RN start date of April 1, 2020 and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within five years from the last day of the month of the RN start date. All entry-level associate degree registered nurses with a RN start date prior to April 1, 2020 must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program as stipulated by the degree requirement program in place at time of hire.
  • Excellent communication skills (verbal and written).
  • Experience working in a team environment.
  • Computer skills required, prior experience with electronic medical record systems preferred.
  • Ability to work flexible hours, which may include days, evenings, nights, holidays, and weekends and on-call.
  • Ability to adapt to unpredictable situations within the work setting.
  • Demonstrated leadership, effective communicator, and excellent critical thinking skills.
  • Current RN license by applicable state requirements.
  • Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross.
  • Additional state licensure(s) and/or specialty certification/training as required by the work area.

Nice To Haves

  • Experience or interest in geriatrics, primary care, or community-based nursing preferred
  • One year of RN experience in an applicable care setting or one year of current LPN experience at Mayo Clinic is preferred.
