About The Position
Community Admissions RN. FT 8a-4p or (4) 10 hour shifts. (1) weekend every (4) weeks. An Admissions Registered Nurse is responsible for presentation of Hospice services to clients who have been referred for all levels of care (general inpatient, home care, and nursing home). Facilitates and completes the admission process after reviewing patient/family goals of care and identifying the appropriate level of care. Initiates individual professional IDT plan of care. Collaborates with the IDT, physician, and facility staff to determine start of care orders. Provides in-services for hospital staff and the community.
