About The Position

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
• Ability to work well within a team environment and collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Strong communication skills for 1-1 patient interactions and collaboration with healthcare professionals.
• Ability to work independently and manage day-to-day activities including communication and meetings.
• Time management skills to manage visits, documentation, and meetings effectively.
• Ability to learn and utilize an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.

Responsibilities
