Mark Twain Health Care District dBa Valley Springs Health & Wellness Center is seeking a Registered Nurse (RN) to work Part-time (2-3 days per week). This position will develop and manage a Patient Chronic Care Management Program . This is a new service in which Medicare outpatients are provided with a treatment plan that is coordinated, communicated, documented, and billed each month. Applicants should have excellent communication skills, computer skills, familiarity with programs such as Excel, prior EMR experience, good clinical judgement, and be able to operate independently. Applicants should be able to multitask, interpret clinical data, make real-time clinical decisions, and provide data reporting. This position will be available March 2, 2026. The Registered Nurse (RN) works under the supervision of the Clinic Manager. As a member of the health care team, the RN performs nursing duties in their capacity for the care of patients in a primary care outpatient setting, including: Assists medical providers in the treatment of patients Supervises and coordinates back office Medical Assistants to ensure patient flow Case management Phone triage Communicate effectively Accurately charts patient problems using electronic health records system Maintains patient records Follows/implements departmental and administrative manuals, standards of practice, and other department protocols Follows contract compliance requirements for specific services, nursing practice regulations, and other pertinent information, all in accordance with HIPAA requirements. Ensures referrals for outside medical services are completed in a timely manner by referral clerk Qualifications The ideal candidate will have an enthusiastic, positive outlook, professional attitude and a willingness to learn and grow. Minimum AS Degree in nursing, Bachelor's preferred Current Active California Registered Nurse license without restrictions Three (3) years of experience as a Registered Nurse in active patient care nursing; One (1) year of ER/Urgent Care preferred Strong assessment skills Excellent communication skills; works cohesively with providers and other medical staff Case management Current Healthcare Provider CPR certificate Strong computer skills required Prior experience with Electronic Health Records preferred Prior supervisory experience preferred Understanding of Title 22 clinic regulations Bi-lingual preferred, but not required This position requires walking, standing, and sitting for prolonged periods of time; stooping, kneeling, crouching, and reaching; pushing, pulling and lifting up to 30 pounds; raising objects overhead; manual dexterity for operation of computer, phones and small office machines; hearing, talking, and good verbal and written communication skills; repetitive motions of the wrists, hands, and fingers. This position requires the ability to safely operate a motor vehicle. Employment is contingent on passing a (post-offer) background check and drug test.
