Requirements

  • Licensed Registered Nurse (RN) in DC
  • Minimum of 2 years of nursing experience required
  • Strong clinical, communication, and time management skills
  • Ability to work independently and manage a flexible schedule

Nice To Haves

  • Home health experience preferred, not required
  • Knowledge of Medicare/Medicaid guidelines is a plus

Responsibilities

  • Provide skilled nursing care in patients’ homes
  • Perform patient assessments and develop individualized care plans
  • Administer medications and treatments as ordered
  • Monitor patient conditions and communicate updates to physicians
  • Educate patients and caregivers on disease management and care
  • Complete accurate and timely EMR documentation
  • Collaborate with interdisciplinary care teams
