A DAY IN THE LIFE OF A STRONG FAMILIES OF VERMONT REGISTERED NURSE This position works with pregnant people (does not have to be first child) and new parents through the transition to parenthood, with continuing support until the child celebrates their second birthday. This Program supports healthy decision making and effective parenting through the joys and challenges of being a parent or caregiver. Responsibilities include but are not limited to:

Requirements

  • Graduation from an accredited school of nursing required. Bachelors of Science in Nursing required.
  • Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency.
  • Experienced with electronic medical records required.
  • Valid Vermont Driver's License and car insurance.

Nice To Haves

  • Experience in community maternal child and family health, and public health nursing preferred. Prior Home Care experience preferred.

Responsibilities

  • Assists individual and family to identify goals and actions that will assist them in meeting their desired outcomes, e.g., increased knowledge and skill acquisition, improvement in health status, awareness of community resources
  • Develops and maintains community partnerships to support appropriate client referrals to CIS and a strong CIS system of care
  • Seeks and encourages input from CIS team members and other community providers in determining the priority health care needs of the maternal child and family health population
  • Provide nursing assessments, life-skill, prenatal, and child-care education for prenatal and new mothers.
  • Creates, maintains and supports a safe environment for open discussion with families
  • Assists in creating positive work environment for open discussions with families
  • Participates in the review and analysis of the CIS reports; identifies areas of achievement and areas for improvement
