About The Position

Vighter is seeking compassionate and experienced Registered Nurses (RNs) to join our team at the Office of Community Care (OCC) in Glendale, Colorado. As a Case Manager / Utilization Review Nurse, you’ll play a key role in coordinating and reviewing healthcare services for veterans, ensuring they receive timely, high-quality care through the VA network. Requirements

Active, unrestricted RN license (Colorado or compact state).
Minimum 2 years of RN experience within the past 3 years — Case Management, Utilization Review, or Primary Care preferred.
Strong knowledge of clinical documentation, care coordination, and medical necessity guidelines.
Ability to work independently and exercise sound clinical judgment.
BLS certification (AHA, ARC, or ASHI).
Excellent communication and organization skills.

Nice To Haves

Experience with VA systems or government healthcare programs a plus.

Responsibilities

Conduct clinical reviews of medical records to determine the appropriateness, level, and continuity of care.
Collaborate with providers and interdisciplinary teams to ensure patients receive high-quality, cost-effective care.
Coordinate referrals, authorizations, and follow-up care for veterans within the VA and community care network.
Apply evidence-based guidelines and utilization criteria to evaluate the necessity of services.
Communicate clearly and compassionately with patients, families, and healthcare teams.
Support preventive health programs, immunizations, and patient education when needed.
Ensure documentation complies with VA, HIPAA, and Joint Commission standards.
Participate in quality improvement initiatives that enhance patient outcomes and care delivery efficiency.
