About The Position

Four K Health Care is seeking a Part-Time Registered Nurse (RN) to provide annual nursing assessments and post-hospitalization visits for our clients in the San Antonio and New Braunfels areas. This role is ideal for a nurse who values autonomy, meaningful client relationships, and community-based care.

Requirements

• Active Registered Nurse (RN) license in the state of Texas
• Graduation from an accredited nursing program
• Current CPR/BLS certification
• Strong assessment, communication, and documentation skills
• Ability to work independently in a home-based setting
• Reliable transportation and valid driver's license

Nice To Haves

• Experience in home health and/or disability services preferred

Responsibilities

• Conduct annual nursing assessments in clients' homes
• Complete post-hospitalization visits and follow-up evaluations
• Assess health status, identify changes in condition, and report findings
• Review and update care plans as needed
• Provide education to clients, families, and caregivers
• Maintain accurate and timely documentation
• Ensure compliance with agency policies and state regulations
• Advocate for client safety, dignity, and independence
