About The Position

The Registered Nurse (RN) plays a key role in ensuring the health, safety, and well-being of our residents through on-site clinical support, staff collaboration, and adherence to regulatory standards. The RN provides assessment, care coordination, staff training, and ongoing oversight of resident care needs. This role requires professionalism, compassion, strong communication skills, and the ability to work collaboratively with residents, families, and team members.

Requirements

  • Current Registered Nurse (RN) license in the State of Wisconsin (required)
  • Minimum one year experience in assisted living, home health, or long-term care preferred
  • Strong organization, follow-through, and independent work skills
  • Compassionate, resident-focused approach to care
  • Strong judgment, problem-solving, and decision-making skills
  • Excellent verbal and written communication skills

Nice To Haves

  • Experience using a computerized resident record system preferred
  • Experience working with seniors

Responsibilities

  • Completes initial assessments of potential residents to ensure needs can be met
  • Completes resident assessment within two weeks of admission
  • Ensures residents maintain admission criteria; reassesses and recommends alternative placement if needed
  • Collaborates in creating service agreements for new residents; reviews and updates plans as needed and at least annually
  • Ensures service agreements are current and compliant with state regulations
  • Monitors and maintains resident charts and clinical progress notes
  • Communicates resident condition changes to physicians and appropriate personnel (including county caseworkers)
  • Facilitates coordination of services with external providers
  • Oversees medications and renewals; follows up with providers as needed
  • Ensures physician orders are signed and implemented within 24 hours
  • Ensures medication administration procedures are followed and addresses corrections or errors with staff
  • Ensures new medication sheets are prepared monthly
  • Reviews resident medication records monthly for accuracy and signatures
  • Documents medication disposal per procedure
  • Conducts supervisory visits 14 days after admission and at least every 90 days thereafter
  • Collaborates as an active member of the resident care team
  • Provides health training and ongoing education to staff; documents training per regulations
  • Oversees medication practices and medical equipment use
  • Ensures staff understand and follow resident care procedures
  • Monitors accuracy of vitals and monthly health indicators as requested
  • Shares resident and family concerns with appropriate team members
  • Assists in development and implementation of policies and procedures
  • Attends staff meetings and participates in team communication
  • Supports emergency preparedness procedures
  • Adapts to changes within the assisted living industry
  • Promotes a positive, team-focused work environment
  • Works flexible hours as needed and participates in clinical on-call support
  • Supports compliance with regulatory standards and survey readiness
