About The Position

We are seeking a compassionate and highly skilled Registered Nurse (RN) to join our team in a part-time capacity, supporting both hospice care and clinic-based home site visits. This role requires a nurse who is clinically strong, independent, and comfortable delivering care in patients’ homes.

The ideal candidate will have strong experience in wound care, IV therapy, and direct patient care, with a commitment to providing high-quality, patient-centered services.

Requirements

• Active Registered Nurse (RN) license in [State]
• Minimum 1–2 years of nursing experience (home health or hospice preferred)
• Strong proficiency in: Wound care management
• IV insertion and therapy
• Patient assessment and clinical decision-making
• Experience with home visits or hospice care is highly preferred
• Ability to work independently and manage time effectively
• Excellent communication and interpersonal skills
• Reliable transportation for home visits

Responsibilities

• Provide skilled nursing care to patients in hospice and home settings
• Perform comprehensive patient assessments and develop care plans
• Deliver and manage wound care treatments (including advanced wound care as needed)
• Initiate and maintain IV therapy, including medication administration
• Monitor patient conditions and report changes to physicians and care teams
• Educate patients and families on treatment plans, medications, and care techniques
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary teams to ensure continuity of care
• Ensure compliance with all clinical, regulatory, and safety standards
