More about our team Portage Pointe is a beautiful Eden Alternative home for 60 Elders from our community. With 5 different homes, a Registered Nurse (RN) can have a positive impact on the lives of Elders through supporting the CNA’s who provide direct Eldercare and positively impacting the lives of Elders through the application of critical thinking skills. This unique environment enables nurses to provide the highest quality patient care. Hone your physical assessment skills even further through providing direct care to Elders by using the nursing process within the scope and standards of practice in our long-term care unit. Because nurses lead the care team, interested candidates must possess exemplary customer service fitting of our high standards and environment of care. Requirements

  • Applicants should have a current Michigan RN license and possess an associate degree from an accredited nursing school (bachelor's degree preferred).
  • Basic Life Support certification (available upon hire)
  • Annual Influenza Vaccination or Declination Required

Nice To Haves

  • 1-2 Years Medical/Surgical Experience (Preferred)

Responsibilities

  • Develops and implements a patient focused nursing plan of care in collaboration with the patient/family.
  • Cares for patients in accordance with established best practices.
  • Documents patient care given.
  • Monitor, record, and communicate patient condition as appropriate to care team, physician, patient, and family.
  • Assist patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
  • Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • Maintains the privacy and security of all confidential and protected health information. Use and disclosure only that information which is necessary to perform the function of the job.
  • Contributes to and assists in maintaining a pleasant patient environment in a safe and orderly manner.
