About The Position

Gateway Post Acute is a premier skilled nursing facility in Porterville, CA, proudly serving the community for over 40 years. We are currently seeking a dedicated and compassionate Registered Nurse (RN) to join our nursing team and help deliver high-quality, resident-centered care. The Registered Nurse (RN) is responsible for supervising day-to-day nursing activities, providing direct patient care, and ensuring that nursing services are delivered in compliance with federal, state, and local regulations, as well as facility policies and procedures. This position works under the direction of the Director of Nursing (DON).

Requirements

  • Graduate of an accredited nursing program
  • Minimum 1 year of nursing experience preferred
  • Current, active California Registered Nurse (RN) license in good standing
  • Current BLS/CPR certification
  • Strong leadership, communication, and organizational skills
  • Ability to read, analyze, and interpret medical and regulatory documentation
  • Basic computer proficiency and experience with EMR systems preferred
  • Ability to work effectively in a fast-paced, team-oriented environment
  • Ability to lift up to 25 pounds regularly and 100 pounds occasionally with assistance
  • Frequent standing, walking, bending, pushing, and pulling
  • Ability to cope with mental and emotional stress
  • Exposure to blood-borne pathogens, chemical agents, and other clinical hazards
  • Must meet facility health requirements, including physical exam and TB screening

Nice To Haves

  • Long-term care or skilled nursing experience preferred
  • Basic computer proficiency and experience with EMR systems preferred

Responsibilities

  • Supervise and coordinate day-to-day nursing activities during assigned shift
  • Provide direct nursing care, including medication administration and treatments
  • Assess residents’ conditions and report changes promptly
  • Participate in the development, implementation, and review of resident care plans
  • Ensure compliance with all facility policies, procedures, and regulatory requirements
  • Monitor and document resident status, care provided, and clinical outcomes
  • Complete and review nursing documentation, incident reports, and audits
  • Make rounds with physicians and ensure orders are carried out accurately
  • Assist the DON with staffing, performance evaluations, and staff development
  • Participate in care conferences, quality assurance activities, and surveys
  • Maintain a safe, clean, and therapeutic environment for residents and staff
  • Respond to emergencies and support resident safety at all times
