Our Home Health RN Case Managers enjoy some notable benefits: Up to $80,000 Annual Salary 1:1 nurse-to-patient ratios where you impact outcomes Flexible assignments, autonomy and work-life balance Online training, growth and ability to earn CEUs Tuition discounts through Rasmussen University Health, Dental, Vision, PTO, Free Learning Courses, Competitive Pay, 401K, Life Insurance As a Home Health RN Case Manager, here’s a big-picture view of what you’ll do: Complete accurate and timely OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments Develop, implement, and update individualized patient care plans Perform skilled nursing visits including assessments, medication management, wound care, and patient education Coordinate care with physicians, therapists, and interdisciplinary team members Ensure compliance with Medicare, state, and agency regulations Maintain thorough and timely clinical documentation in the EMR Provide education and support to patients and caregivers A few must-haves for Home Health RN Case Managers: Graduate of an accredited nursing program and active RN license in Kansas CPR certification Knowledge of state and federal home health regulations Clinical proficiency in nursing specialty, strong communication skills and compassion Why Work for Interim HealthCare? Requirements

  • Graduate of an accredited nursing program and active RN license in Kansas
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills and compassion
