About The Position

Summit Home Care is searching for an experienced and motivated Registered Nurse (RN) to join our fast-growing, innovative post-acute Home Health team committed to supporting both clinicians and patients. At Summit, we offer AI-Assisted clinical documentation to streamline charting, company-covered wound care certification and free CEUs. Full-Time Registered Nurse (RN): Hours: Monday - Friday; 9:00 AM - 5:00 PM (40 Hours/Week) Travel Location/Territory: Nora, IN & Surrounding Areas (Mileage Reimbursed at $0.48/Mile) Base Salary: $100,000/Year Sign on Bonus: $10,000 (half paid at 6 months, additional half paid at 12 months) Additional Earnings: $50 per Point Over 45 Points Weekly (visits are weighted 1-3 points/visit

Requirements

  • Graduate from an accredited school of nursing, associate or bachelor’s degree.
  • Valid/active nursing license.
  • Must be a licensed driver with an automobile in good working order and insured in accordance with the organization's requirements.
  • Valid CPR certification is required.
  • Recent TB test, required.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending, kneeling or crouching.
  • Must be able to lift, position or transfer patients and lift supplies and equipment.
  • Must have visual acuity and hearing to perform required nursing skills.

Nice To Haves

  • Prior home health care or acute care experience preferred.
  • Management experience preferred but not required.

Responsibilities

  • Completes an initial assessment of patient and family to determine home care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician.
  • Provides direct patient care as defined in the State Nurse Practice Act.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per the assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Acts as Case Manager when assigned by Clinic Manager and assumes responsibility to coordinate patient care for assigned caseload.
  • Prepare clinical notes and update the primary physician when necessary and at least every 60 days.
  • Communicate with the physician regarding the patient's needs and report any changes in the patient's condition; obtain/receive physician's orders as required.
  • Communicate with community health related people to coordinate the care plan.
  • Participate in on-call duties as defined by the on-call policy.
  • Ensure that arrangements for equipment and other necessary items and services are available.
  • Instruct, supervise, and evaluate home health aide care provided every two (2) weeks.
