About The Position

We are currently seeking a PRN Home Health Registered Nurse (RN) to join our dedicated team. In this role, you will provide care primarily within senior living campuses, ensuring residents receive exceptional, personalized care in a compassionate and supportive environment. As a Home Health RN, you will be responsible for assessing, coordinating, and managing the overall care of patients in their homes or senior living communities through Covenant Home Health, which is primarily operated by the parent organization Fellowship Square, Christian Care. This partnership reflects our shared mission and values of providing compassionate care and dedicated support to our residents. Care is provided 90% of the time on senior living campuses, where you will collaborate closely with an interdisciplinary team to develop and implement care plans that ensure the individual needs of each patient are met. Now Hiring: PRN and Full-Time RN – Senior Living Communities (West/Central Valley)We are currently seeking a dedicated full-time Registered Nurse to provide services at Senior Living Communities in the West and Central Valley regions.

Requirements

  • Graduate of an accredited School of Nursing
  • Active Registered Nurse (RN) licensed in Arizona with current valid credentials.
  • Minimum two (2) years’ Registered Nursing experience in Home Health required.
  • Must pass background check and meet all health and safety requirements.
  • Valid driver’s license and reliable transportation required.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently, manage time effectively, and provide compassionate care to seniors.
  • Ability to travel to designated service areas, including West Valley to Central Phoenix or Central Phoenix to East Valley to provide care to patients within their homes or Senior Living Communities.

Responsibilities

  • Case Management: Develop and implement individualized care plans for patients, coordinating with physicians, family members, and other healthcare providers to ensure optimal care.
  • Patient Assessment: Conduct thorough assessments of patients’ physical, emotional, and social needs and update care plans as required.
  • Care Coordination: Ensure the timely delivery of care services, including scheduling visits and coordinating with other healthcare professionals.
  • Medication Management: Oversee medication administration, monitor for side effects, and educate patients and families on proper medication use.
  • Patient Education: Provide education on health management, disease prevention, and improving quality of life for patients and their families.
  • Documentation: Maintain accurate and up-to-date records for all patient visits, including care plans, progress notes, and communication with the care team.
  • Advocacy: Act as an advocate for the patient’s needs, ensuring that care is delivered in accordance with the patient’s preferences and goals.
  • Collaboration: Work closely with physicians, therapists, and other care team members to ensure a coordinated approach to patient care.
