Requirements

• Current RN license in Connecticut
• Physical exam within last 6 months
• TB test within 12 months & CPR certification
• Valid driver’s license & automobile insurance
• Ability to attend 1–3 day orientation at Stamford office

Nice To Haves

• OASIS and WellSky (Kinnser) experience preferred
• Med-surg and wound care experience a plus

Responsibilities

• Conduct patient admissions and develop individualized care plans
• Gather medical history and perform physical assessments
• Monitor patient status, vitals, and progress in the home
• Deliver skilled nursing care in compliance with HIPAA
• Evaluate and adjust care plans as needed
• Collaborate with the interdisciplinary team for comprehensive care
• Coordinate services with LPNs, HHAs, and therapy providers
• Document visits and submit clinical notes promptly
