Requirements

  • Active Maryland Registered Nurse (RN) license or multi-state licensure
  • CPR / First Aid certification
  • Reliable transportation for home visits
  • Strong assessment and communication skills

Nice To Haves

  • Experience in home care, hospice, or geriatric nursing preferred

Responsibilities

  • Conducting in-home client assessments and reassessments
  • Providing clinical oversight of CNAs, GNAs, and HHAs
  • Monitoring client status and completing follow-up visits
  • Developing and updating individualized care plans
  • Communicating with families, caregivers, and healthcare providers
  • Supporting infection control and safety best practices
  • Maintaining accurate documentation in accordance with regulations
  • Participating in caregiver education and support as needed
