Requirements
- Active Maryland Registered Nurse (RN) license or multi-state licensure
- CPR / First Aid certification
- Reliable transportation for home visits
- Strong assessment and communication skills
Nice To Haves
- Experience in home care, hospice, or geriatric nursing preferred
Responsibilities
- Conducting in-home client assessments and reassessments
- Providing clinical oversight of CNAs, GNAs, and HHAs
- Monitoring client status and completing follow-up visits
- Developing and updating individualized care plans
- Communicating with families, caregivers, and healthcare providers
- Supporting infection control and safety best practices
- Maintaining accurate documentation in accordance with regulations
- Participating in caregiver education and support as needed
